1. Completed APPLICATION FORM for "Registration of Death and issuance of No Objection Certificate for transportation of mortal remains (Body/Ashes) of the deceased to India". To download the form, Click here.

2. Fill in all the information correctly. Incomplete applications will be returned without service.

3. DOCUMENTS REQUIRED IN RESPECT OF THE DECEASED:

a. Original Indian Passport (FOR INDIAN CITIZENS) along with a photocopy of pages containing personal particulars (first three and last two pages) of the deceased and copy of Legal Status proof copy in USA.

b. USA Passport copy (FOR NON-INDIANS ONLY) and Original OCI Card of the deceased and along with a photocopy of pages containing personal particulars (first three and last two pages) of the deceased.

c. Indian Passport/OCI Card will be cancelled and returned to the applicant. Keep it safe.

d. Death Certificate issued by USA authorities (in original and a photocopy).

e. Embalming Certificate (in original and a photocopy) stating that the body has been embalmed in accordance with the rules and regulations of international shipping and that the body has been placed in a hermetically sealed container with zinc liner and wooden outer container.

f. Certificate from the Department of Health stating that the deceased did not have any communicable/contagious diseases (Non-communicable Disease certificate), in original and a photocopy.

g. Burial/Transit Permit (in original and a photocopy).

h. Cremation Certificate - in case of Ashes (in original and a photocopy).

i. Last Local USA Address proof of the deceased – copy only.

j. Other relevant documents which needs to attested by the Consulate.

4. DOCUMENTS REQUIRED IN RESPECT OF INFORMAT WHO IS PROCESSING THE DOCUEMNTS ON BEHALF OF THE DECEASED:

a. Copy Indian/USA Passport copy of the informant. 

b. Copy Legal status proof in USA of the informant.

c. Local Address proof of the informant.

d. Authorization letter from immediate family member(s) of the deceased, authorizing the INFORMANT, to process the documents on their behalf (in case the INFORMANT is not the immediate family member).
