Life Certificate for Power of Attorney /Registration of Property/Banks

Required documents:

1.  Please fill in the application form for Life Certificate with one recent passport size photo with white background affixed in the box. 2.   If applications are being sent by postal mail. All copies of supporting documents should be NOTARIZED. If applicants are coming in person to consulate for any Consular services, then there is no need of notarization, but applicants need to bring all original documents.

3.  Applicant's valid Original Passport (Indian or USA Passport) and photocopies of pages containing personal particulars (first three and last two pages of passport).

4.  Copy of valid USA Visa/Green Card for Indian Passport holders and Copy of OCI Card (First and Last pages of personal information) for USA passport holder of Indian origin.

5.  Copy of valid Proof of address (Utility Bill, Driver License or Lease Agreement).

6.  If the applicant is holding a B1/B2 Visa:

      1. a.  He/She should provide his/her Son/Daughter/Relative's current Passport copy (containing personal information).
      2. b.  Son/Daughter/Relative's Driver's License copy.
      3. c.  If your Son/Daughter/Relative is a US citizen then his/her old Indian Passport Copy (First and Last pages of personal information).
      4. d.  A letter from him/her to the Consular Officer requesting this service for his/her Parent(s)/Relative.
        Applicants applying for Life Certificate other than pension may provide reasons for the same in a self-attested document.

8. All photocopies of required documents must be legible (readable) and clear copies. Blurred/dark/illegible copies may not be accepted.

9.  For those Applicants who are unable to come in person to the Consulate and for the applications being sent to the Consulate by postal mail. Please enclose the recent (no later than 15 days) Doctor's Certificate, from a registered Medical Practitioner, along with the application certifying that Applicant (with name) has been examined (with date) by the Medical Practitioner and is alive on the day.

10.  Fee is USD 25.00 in favor of “CONSULATE GENERAL OF INDIA” and USD 2.00 in favor of “ICWF” will be charged.

11.  Cash payment is also accepted at the counter of the Consulate. Applicants are requested to come with exact change for efficient services.

12.  Personal checks, Debit cards and Credit cards are not accepted.

13. If the application is sent by postal mail, please send a separate USD 20.00 Cashier’s Check or Money Order for return mail OR include your prepaid envelopes for return mail.
