B. REQUIRED DOCUMENTS:

  1. Completed application form for "Attestation of Power of Attorney or Civil or Agricultural or Commercial Documents" with one passport size photo affixed in the box. To download the application form CLICK HERE icon-image.

  2. Fill in all the information correctly. Incomplete application will be returned without service.

  3. If applications are being sent by postal mail. All copies of supporting documents (including Power of Attorney or Civil/Agricultural/Commercial Documents) should be NOTARIZED. If applicants are coming in person to consulate for any Consular services, then there is no need of notarization, but applicants need to bring all original documents.

  4. Applicant's valid Original Passport (Indian or USA Passport) and photocopies of pages containing personal particulars (first three and last two pages of passport).

  5. Copy of valid USA Visa/Green Card for Indian Passport holders and Copy of OCI Card (First and Last pages of personal information) for USA passport holder of Indian origin, or Renunciation of Indian Citizenship Certificate, if applicable.

  6. Copy of Proof of address (utility bill, driver license or lease agreement).

  7. Two sets of Power of Attorney or Civil/Agricultural/Commercial Documents to be attested. One for the Consulate (to be submitted along with the application form) and second one for the Applicant.

  8. Name and father's name of the EXECUTANT mentioned in the Power of Attorney or Civil/Agricultural/Commercial Documents should be exactly as in his/her Passport with expanded initials. For example, the document may state that "I (name of applicant), ……… son/daughter of (name of father) …………resident of ……… (address in India and abroad) and holder of Indian/US Passport No. …… issued on …… at …………… PIO/OCI card no ............. issued on ............... at ............... (if applicable) hereby ……………."

  9. All the pages of the Power of Attorney or Civil/Agricultural/Commercial Documents have to be signed by the applicant.
